Sub center:

• Sub center MIES data set (Monthly) : Monthly data entry for the sub center should be done in sub center MIES data set. This data set is based on institutional based reporting i.e. the services provided by the ANM/FHW of the sub center are to be reported.

• Line listing Birth data set (Monthly) : All the births that have had taken place in the sub center or home deliveries in the village ( conducted by ANM/Nurse/Doctor or TBA or Non TBA or dia) are to be reported name wise in the line listing birth data set.

• Line listing Birth Aggregated data set (Monthly) : In case the name wise data for birth is not available then line listing aggregated data set is to be filled. In this male female bifurcation is to be mentioned.

• Line listing Death data set (Monthly) : All deaths that take place in the village (which have not taken place or not attended by any institution except for sub center) are to be reported in this form. The age category and probable cause of death should be filled from the option given in the foot notes of the form.

• Line listing Maternal Death data set (Monthly) : All maternal deaths taking place in the village (which have not taken place or not attended by any institution except for sub center) are to be reported here. The period of death and the cause of death are given in the foot notes of the form and are to be filled in accordance.

Primary Health Center:

• PHC MIES data set(Monthly) : Monthly data entry for the PHC should be done in PHC MIES data set. This data set is based on institutional based reporting i.e. the services provided by the PHC and its staff, in the PHC building is to be reported in this form.

• Line listing Birth data set (Monthly) : All the births that have had taken place in the PHC labor room or operation theater are to be reported name wise in the line listing birth data set.

• Line listing Birth Aggregated data set (Monthly) : In case the name wise data for birth is not available then line listing aggregated data set is to be filled. In this male female bifurcation is to be mentioned.

• Line listing Death data set (Monthly) : All deaths that take place in the PHC (inside the building) are to be reported in this form. The age category and probable cause of death should be filled from the option given in the foot notes of the form.

• Line listing Maternal Death data set (Monthly) : All maternal deaths taking place in the PHC are to be recorded here. The period of death and the cause of death are given in the foot notes of the form and are to be filled in accordance.

Community Health Center:

Sub Divisional Hospital/District Hospital.

• DH-SDH MIES data set (Monthly) : Monthly data entry for the DH/SDH should be done in DH/SDH MIES data set. This data set is based on institutional based reporting i.e. the services provided by the hospitals and its staff, in the respective building is to be reported in this form.

• Line listing Birth data set (Monthly) : All the births that have had taken place in the SDH/DH labor room or operation theater are to be reported name wise in the line listing birth data set.

• Line listing Birth Aggregated data set (Monthly) : In case the name wise data for birth is not available then line listing aggregated data set is to be filled. In this male female bifurcation is to be mentioned.

• Line listing Death data set (Monthly) : All deaths that take place in the SDH/DH (inside the building) are to be reported in this form. The age category and probable cause of death should be filled from the option given in the foot notes of the form.

• Line listing Maternal Death data set (Monthly) : All maternal deaths taking place in the SDH/DH are to be recorded here. The period of death and the cause of death are given in the foot notes of the form and are to be filled in accordance.
  

Block Data set:

• Block aggregation MIES dataset (Monthly) : is to be filled if the data entry is to be done from block level onwards, i.e. below block level facilities are not doing data entry. In this form all the births and deaths are to be entered in figure. No separate data entry is to be done for birth, death and maternal death .This form is aggregation of services provided by all the facilities under the block.

District Data sets:

• District MIES Dataset (Monthly) : is to be filled if the data entry is to be done at district level, i.e. below district level facilities are not doing data entry. In this form all the births and deaths are to be entered in figure. No separate data entry is to be done for birth, death and maternal death. This form is aggregation of services provided by Sub center, PHC, CHC and SDH/DH and private institutes under that district.

• District Stock Data set (Monthly) : Entry of stocks is to be done at district level only. (even if data entry is done by PHC/SC/CHC/block the stock dataset will be filled at only district stock).

• FMR Data set (Monthly) : is to be entered by district level staff; it should be compilation of all the expenditure done by different facilities below district. All the value should be entered in Lakhs. Note: For Apr08-Mar09 is quarterly and From Apr09 onwards its monthly.

• District Quarterly data set (Quarterly) : is to be entered by the district at the end of every quarter. During the quarter performance is to be filled in this form.

• Annual data set (Annual) : is to be filled by the district level staff at the end of the financial year.

State Data set:

• State FMR Data set (Monthly) : is to be entered by state level staff; it should include only state level expenditure i.e. District level expenditure should not be added in this. All the expenditures should be entered in Lakhs. Note: For Apr08-Mar09 is quarterly and From Apr09 onwards its monthly.

• Quarterly state MIES data set (Quarterly) : is to be entered by the state at the end of every quarter. During the quarter performance is to be filled in this form.

• Annual state MIES data set (Annual) : is to be filled by the state at the end of the financial year.
